Registration Requirements
Please bring with you:
- Child's original Birth Certificate
- Your picture ID
- Physical Exam (done within the last year)
- Florida 680 Immunization Form
- 2 proofs of Residency
a. mortgage or lease
b. current utility bill, income tax record, or current government benefits letter
If you do not have a lease or deed in your name a Verification of Address must be obtained. (Information available on Student Service’s website.)
- Certificate of Eligibility from 4C (will begin issuing Certificates February 1, 2013), you may obtain a certificate by:
- Contacting the 4C office and scheduale an appointment at 321-219-6300
- Going to the 4C office any WEDNESDAY as a walk in from 8:00am to 3:30pm with required documentation
- Attend a VPK Round Up on:
- Saturday, March 2, 2013 from 9:30am-1:00pm OR
- Wednesday, June 5, 2013 from 5:00pm-8:00pm with required documentation.
- EARLY LEARNING INFORMATION SYSTEM (ELIS)
Beginning Spring/Summer 2013 get your VPK certificate online using
Florida’s new Early Learning Information System (ELIS). ELIS is a
comprehensive web-based system that will allow families 24/7 access
to services provided by the Early Learning Coalition. Please visit
